New Patient Forms
What to Expect
On your first visit you can expect to begin by filling out some brief paperwork. This paperwork provides us with general information about yourself, medical history, and your current condition. You have the option to download and complete the forms above and bring them with you to your first appointment, or come in early and we will provide the paperwork for you to fill out at the clinic. We will also make a copy of your insurance card and verify your chiropractic benefits for you.
Once the paperwork has been completed, Dr. Chad will spend some time getting to know you and the reason for your visit. He will then determine if chiropractic care is right for you. This is a free consultation.
History and Examination
If it is determined that chiropractic care is right for you, a thorough history will be performed. The history involves multiple questions related to your condition. Next a chiropractic examination will be performed. The goal of the examination is to determine the root cause of your problem. The examination typically involves postural assessment, range of motion, strength testing, neurological testing, and orthopedic testing.
Based on your history and the results of the examination a set of x-ray's may be performed. These x-rays are used to rule out more serious conditions, check joint structure and alignment, and help aid the doctor in developing a proper treatment plan for you.
Chiropractic adjustments, muscle work such as stretching, or additional therapies typically are performed on the first appointment unless more serious conditions need to be ruled out. The doctor may decide to wait until the second visit for treatment based on the findings of the examination or x-rays.
Length of First Visit
The first visit typically takes about 1 hour.
Your Second Visit
The second visit typically occurs within 1-2 days after the first visit. On the second visit, Dr. Chad will begin by addressing any questions you may have after your first appointment. He will also explain your diagnosis. This is called the review of findings. This review will explain the cause of your particular condition. We never want a patient to leave without having a clear understanding of what their problem is, and what therapies it will take to get them better. A treatment plan, goals of care, and the approximate amount of time it will take for you to heal will also be presented. After the review of findings you will know what to expect in the days ahead. You will also be notified of what you have for insurance coverage, or what out of pocket expenses you may be liable for.
The treatment typically consists of specific chiropractic adjustments to restore function of your joints, stretching, deep tissue work, and occasionally electric muscle stimulation therapy. At home stretching and strengthening exercises will also be recommended to help you recover quicker. Stress management, lifestyle changes, postural awareness, ergonomics, and nutrition are also topics that will be discussed on an individual basis.
